Submitting an Application
If you already have a maintenance order or agreement, just follow these easy steps to submit an application to start the enrolment process.

To complete the enrolment process you may need to send in forms about the amounts due and received under your maintenance order or agreement.Find Forms

3. Print, fill out & mail it to

If you can, please include a copy of your maintenance order or agreement with the completed enrolment application.

To submit an enrolment application online you’ll need to have a FMEP web account.
If you don’t have a web account you will need to create one. Once you’ve created your account go to Enrol Now and follow the steps to submit your application online.
If you already have a web account you will need to sign in. Once signed you’ll find the application under Account Tools. Complete the steps and then submit your application online.
